Why so few fields? Section 6(1) limits consent to the personal data necessary for the stated purpose. The stated purpose here is "send you a PDF". That needs an email and nothing else — so nothing else is demanded, and anything you declined on the notice isn't even rendered. A form that makes the checklist conditional on your phone number isn't collecting free consent, it's collecting a toll.
